Chronic Disease Summit 2021
Managing chronic disease is one of the biggest challenges facing Australian GPs today.  Join Australia’s leading experts as they discuss diagnosis, treatment options and management of chronic disease in Australia. 

This program will focus on five of the top ten chronic conditions including Cardiovascular, Stroke, Diabetes, Obesity and Chronic Pain as well as the most rapidly emerging condition, Neurological Disease Management. Attendees will hear from renowned local specialists and have hands on experience with recent advancements in medical technology associated with these chronic conditions.

This education is a CPD Activity under the RACGP CPD Program - worth 12 CPD points.

LEARNING OUTCOMES:

Diagnosis:
Identify the chronic disease through understanding of advancement in diagnostic technologies.

Prevention: 
Explain strategies for patients with chronic conditions to prevent or slow deterioration.

Management: 
Articulate therapies to patients with chronic disease to manage a worsening condition.

Referral: 
Identify when the disease progression pathway requires referral to a specialist for ongoing management and
understand clinical signs, symptoms and patient management considerations
post specialist intervention when they return for ongoing management.
FACULTY
CARDIOVASCULAR DISEASE MANAGEMENT
PROF DARREN WALTERS
Prof Walters is a structural interventional Cardiologist and has been one of the pioneers of structural interventions in Australia such as transcatheter aortic valve replacement and percutaneous mitral valve repair and replacement. He is Director of Cardiology at The Prince Charles Hospital and Visiting MedicalOfficer at St Vincent’s Brisbane (St Vincent’s Northside Hospital).

DR PETER LARSEN
Dr Peter J Larsen is a Clinical and Interventional Cardiologist with special interests in interventional cardiac and endovascular procedures, including coronary angioplasty and stenting, peripheral arterial disease diagnosis and treatment, percutaneous valvuloplasty and structural heart interventions. He is also a registered specialist in CT Coronary Angiography.
DR HIMABINDU SAMARDHI
Dr Himabindu Samardhi is a Clinical Cardiologist and Cardiac electrophysiologist. Himabindu is an associate at CVC CardioVascular Clinics and Staff Cardiologist and Electrophysiologist at The Royal Brisbane Hospital. Her interests include all aspects of Electrophysiology in tertiary care inclusive of complex ablations for atrial fibrillation and ventricular tachycardia, device implantation and inherited heart rhythm disorders. 

DR IAN CAMPBELL
Dr Ian Campbell is a Vascular and Endovascular surgeon with extensive training in open and endovascular surgery. Clinical interests include: Varicose veins, Deep vein thrombosis and superficial thrombosis, Aortic aneurysms, lower limbs and artic/iliac arterial disease, diabetic foot disease, carotid stenosis, renal access including fistula creation and maintenance & wound care.
CHRONIC PAIN MANAGEMENT
DR JAMES O’CALLAGHAN
Dr James O’Callaghan is an esteemed pioneer and advocate of pain medicine and multidisciplinary pain management, in Queensland and throughout Australia. In 2014, Dr O’Callaghan received the “Distinguished Member Award” from the Australian Pain Society. He has pioneered interventional pain techniques in Queensland and has been performing advanced techniques such as radiofrequency neurotomy and spinal cord stimulation for over 30 years.
DIABETES MANAGEMENT
LAURA ZIMMERMAN
Laura is a registered nurse and credentialed diabetes educator as well as a specialist nurse practitioner candidate. She has achieved postgraduate certifications in both Chronic Disease Self-Management Advising and Diabetes Education and Management. She is currently the Chair of the Queensland branch of the Australian Diabetes Educators Association; and is a respected speaker and lecturer on diabetes management.

DR YOGESH RAJE
Dr Yogesh Raje is a Specialist Consultant Endocrinologist and Internal Medicine Physician at Robina Hospital and Gold Coast University Hospital for 7 years. His areas of speciality include – Newer paradigms of treatment in Type 1 and Type 2 Diabetes, Thyroid disorders, Pituitary disorders, Adrenal disorders, Apart from being a busy clinician, he is also actively involved in academics and teaching. He is a Senior lecturer at Griffiths university.
MOVEMENT DISORDERS MANAGEMENT
DR ROBERT ADAM
Dr. Robert Adam is an experienced consultant neurologist with particular expertise in movement disorders and memory problems. He sees patients with all neurological conditions including headache, migraine, tremor, movement disorders, Parkinson’s disease, multiple sclerosis, stroke, epilepsy, seizures and dementia. He works at the Royal Brisbane Hospital and admits private patients to The Wesley Hospital and St Andrew’s War Memorial Hospital.

DR NORMAN MA
Dr Ma is neurosurgical consultant specialist at Royal Brisbane and Women’s Hospital. He also works as a specialist paediatric neurosurgeon at the Lady Cilento Children’s Hospital as a Visiting Medical Officer. He has a special interest in research and teaching, working closely with the University of Queensland and is currently completing a Masters in Philosophy in neonatal research at the University of Queensland Centre for Clinical Research.

OBESITY MANAGEMENT
DR JORRIE JORDAAN
With over 25 years of surgical experience, he is known for his meticulous surgical technique, low complication rate and ethical conduct. He performs minimally invasive surgery for a range of health issues including hernias, reflux, gallstones, thyroid and parathyroid disorders. His primary clinical interests include laparoscopic bariatric surgery for the treatment of weight management and the health complications associated with severe obesity.
DR NOVA JORDAAN
A bariatric medical doctor and surgical assistant, she has spent nearly a decade managing the medical needs of weight loss surgery patients before and after surgery to enhance the safety and outcomes of procedures. Dr Jordaan helps patients achieve significant sustained weight loss coupled with long term quality of life by adjusting medications during the rapid resolution of weight related co-morbidities, troubleshooting side effects, and directing long term nutritional monitoring and supplementation for best outcomes. 

STROKE RISK MANAGEMENT
DR CLAIRE MULLER
Dr Claire Muller is a Neurologist and Stroke Specialist primarily based at the Royal Brisbane and Women’s Hospital (RBWH). She is a member of the Stroke Foundation Clinical Council as a representative of the Stroke Society of Australasia (SSA) and has been elected to the SSA committee twice over the last 2 years. She is a passionate educator of Neurology & Stroke Medicine.
